Aboriginal Community Services (ACS) are a non-profit provider of residential aged care and home and community services. We provide aged care services to our Elders / Clients across South Australia in metro and regional locations.

About the role:

Also known as the Client Services Coordinator, the major objective of this role is to provide CHSP coordination and support for the client to deliver their service plan. This will require working closely with the General Manager, SaH Care Partners, Client Service Coordinators, and Community Service Workers (CSW) to communicate in culturally respect ways, and in their own language were possible.

Responsibilities:

- Intake of new client (coordination)

- Develop, implement, and review client service plans

- Manage client services in accordance with the CHSP guidelines and legislation to promote and achieve positive client outcomes.

- Develop and maintain community service resources to maximise service provision and support clients.





- Assist with referrals from Regional Assessment Services and ACAT in a timely and coordinated manner.

- Develop strong cooperative relationship with other ACS staff / programs and external agencies to deliver seamless and integrated client services

- Provide incident / injury management support to Community Service Worker and agency staff

- Support CSW and agency staff to safely deliver culturally appropriate care and support.

About you:

To be successful:

- You will possess at least two years experience in a similar position.

- You will possess a relevant qualification in community services, though this is not mandatory.

- You will possess excellent communication and organisational skills.

- You will be proficient in the use of Microsoft suite of applications.

- You will have a natural flair to quickly learn new user applications like Alayacare, etc.

- You will have a current national police check or are willing to obtain one.

- You will possess a current unrestricted Driver's License.

- You will have unrestricted work rights in Australia.
