About the Role:
Echuca Regional Health is seeking an experienced and compassionate Social Worker to join our Community Rehabilitation Program (CRP). This is an exciting opportunity to work within a multidisciplinary team, supporting clients to achieve their rehabilitation goals and maintain independence within their community.
As a key member of the Community Rehabilitation team, you will provide person-centred assessment, care planning, counselling, education, advocacy, and service coordination to clients and their families.
Key responsibilities include:
· Conduct comprehensive psychosocial assessments and develop individualised care plans.
· Provide counselling, support, and education to clients and their families.
· Facilitate access to appropriate community services and support networks.
· Work collaboratively with multidisciplinary health professionals to coordinate care.
· Support clients through rehabilitation and transition to community-based services.
· Assist with referral management, program development, and data collection activities.
· Contribute to a positive,
collaborative team culture that reflects ERH's CARE Values.
Qualifications/experience required:
· Qualification in Social Work and eligibility for professional membership.
· Minimum five years' clinical experience, including demonstrated knowledge and skills in rehabilitation.
· Strong assessment, care planning, and case management skills.
