What you’ll do
As a Clinical Care Partner within our Transition Care Program (TCP), you’ll provide intensive clinical care management to older people with complex and changing needs as they transition from hospital back into the community.
Working within a multidisciplinary model of care, you’ll support customers through a time-limited period of recovery, reablement and restorative care, helping them regain confidence, independence and function while reducing the risk of avoidable hospital readmission or premature entry into residential care.
You’ll use your clinical expertise to assess changing needs, coordinate the right supports and facilitate safe transitions across hospital, health and community services. Working closely with customers, families, clinicians and service delivery teams, you’ll ensure care is responsive, evidence-based and centred around each customer’s individual goals.
You’ll be trusted to make a valuable impact by:
- Providing intensive clinical care management for customers with complex, escalated or changing care needs
- Completing clinical assessments and contributing to individualised goals and care plans focused on recovery, reablement and independence
- Coordinating timely services, clinical interventions and supports as customers transition from hospital back into the community
- Working collaboratively with multidisciplinary teams, hospitals, external providers and internal services to achieve positive customer outcomes
- Participating in case conferences and providing clinical advice to support safe, high-quality care delivery
- Identifying and responding to clinical risks, incidents and changes in condition while maintaining high standards of documentation and clinical governance
What you’ll bring
To thrive in this role, you will have:
- Minimum qualification, Diploma of Nursing and current AHPRA registration.
- Demonstrated clinical experience supporting people with complex or changing care needs
- Strong assessment, care planning and clinical case management skills
- Experience working collaboratively within a multidisciplinary healthcare environment
- Ability to recognise clinical risk, respond to changes in condition and coordinate appropriate interventions
- Robust communication, organisation and documentation skills with the ability to manage competing priorities
- Current driver’s licence and ability to travel within the community
We would also love you to have:
- Previous experience within a Transition Care Program (TCP)
- Experience supporting safe discharge and transitions between hospital and community-based services
