Care Connect is seeking a Care Manager to join our Aged Care team on a 12-month maximum-term contract (full time). The role supports a caseload across the Loddon Mallee region with adaptable, primarily work from home and monthly travel to the Abbotsford head office, which is fully accommodated.
You'll assess needs, develop personalised care plans and coordinate services to help older Australians remain independent at home. A solid client focus, planning and record-keeping are essential.
