We are seeking dedicated Aged Care Clinical Assessors to join our multidisciplinary health team, working across Melbourne’s Southern & Eastern suburbs.
In this role, you’ll deliver aged care assessment services to older Australians, enabling them to remain living independently at home.
About You:
Preferred candidates will have:
Experience conducting home support or comprehensive aged care assessments within the single assessment system
Ability to work independently and as part of a team
Strong problem-solving skills and ability to follow relevant guidelines
Current Australian driver’s license with a reliable, fully insured vehicle
Valid National Criminal History Check and Working with Children Check (or willingness to obtain)
A willingness to travel within a 50km radius
What You’ll Do:
Conduct face-to-face assessments to determine eligibility for My Aged Care supports
Review previously assessed clients
Develop client-centered support plans using a wellness and reablement approach
Liaise with referral sources and community organisations to secure supports for clients
Ensure timely, accurate documentation via the My Aged Care online portal
