St Vincent's Hospital Sydney is seeking an End of Life Care Coordinator for the Palliative Care unit in Sydney.
This permanent full time role will lead EOL care coordination across SVHNS, provide guidance on Advance Care Planning and Directives, and support staff wellbeing and governance.
The role requires RN Division 1 with APHRA or a social work degree with AASW eligibility, with 5+ years post-registration experience for nurses or equivalent palliative care experience for social workers.
