Calvary Adelaide Hospital is seeking an experienced Registered Nurse to join the Neurosurgery team as a Clinical Care Coordinator at 0.8 FTE.
The role focuses on clinical care coordination, case management and complex discharge planning, working across multidisciplinary teams to improve patient outcomes and the patient journey.
You'll coordinate care from admission to discharge, partner with surgeons, medical staff and ward teams to ensure safe, timely, and productive transitions.
