Our services include Supported Independent Living (SIL), in-home care, community participation, short-term accommodation, and community nursing.

About the Role
We are currently seeking compassionate and reliable Support Workers to join our Supported Independent Living team in Eagleby. In this role, you will support participant to live as independently as possible while encouraging active engagement in their community.

Key Responsibilities

- Provide support with personal care and daily living activities, including meal preparation, household tasks, and medication reminders
- Deliver high intensity supports, including catheter care and complex personal care needs
- Apply safe manual handling techniques in line with training and organisational policies
- Support participants with complex conditions, including neurological conditions such as Myotonic dystrophy
- Assist participants in accessing the community, attending appointments, and participating in social and recreational activities
- Follow individual Behaviour Support Plans and Risk Assessments to ensure safe, consistent, and high-quality care




- Maintain accurate and timely documentation, including shift notes, incident reports, and data collection
- Work collaboratively with team leaders to implement and review support plans
- Build and maintain professional relationships in line with NDIS and organisational standards
- Demonstrate reliability, accountability, and a person-centred approach in all aspects of your work

About You

- Certificate III in Individual Support, Aged Care, or higher (or willingness to obtain)
- Demonstrated experience in high-intensity support, including catheter care and manual handling
- Experience supporting participants with complex needs, including Myotonic dystrophy or similar conditions
- Experience in mental health
- Nursing experience highly regarded
- Robust communication and interpersonal skills
- A genuine passion for supporting people with disabilities
