The Motor Neurone Disease Association of Western Australia (MNDAWA) delivers person-centred support and specialist care to people affected by MND in Western Australia.
We exist to walk alongside individuals and families through one of the most challenging journeys life can offer, providing practical help, emotional support, and a sense of community at every stage.

About the Role
MND Advisors walk alongside people living with MND and their carers from the time of diagnosis.
They provide vital care coordination and emotional support, adapting their approach to each person's needs and circumstances.
The MND Advisory Service provides information, guidance, emotional support, advocacy, care coordination, education and bereavement support through a person-centred approach.
The service aims to empower people living with MND, their carers and families to make informed decisions, access appropriate supports, and navigate the challenges associated with MND with support, confidence and dignity.
You'll work closely with people living with MND and their carers by:




Conducting regular home visits in the community to deliver person-centred coordination of care services
Supporting and promoting the choices of people living with MND
Coordinating multidisciplinary team meetings as required
Delivering education and upskilling community service providers on the needs of people living with MND
Developing and maintaining strong and enduring networks and relationships with key stakeholders
Assisting people living with MND to navigate the complexity around accessing funding bodies such as NDIS, Support At Home etc
Working closely alongside Support Coordinators to complement the care coordination for NDIS Participants
Participating as part of the multidisciplinary team at the MND clinics at Sir Charles Hospital, St John of God Hospital Midland and Fiona Stanley Hospital
Maintaining records to support effective support service provision, performance analysis and reporting
Participating and contributing to the overall MNDAWA services
About You
You are a compassionate and empathetic professional with a genuine commitment to supporting people and families navigating complex and life-changing circumstances.
You are a skilled communicator who thrives in a fast-paced, purpose-driven setting and can build trusted relationships with clients, carers, health professionals and community services.




You're passionate about empowering people to make informed choices and access the support they need to live well with MND.
You'll bring:
Tertiary, health, disability or community care related qualifications
Excellent interpersonal skills
Proven written and verbal communication skills
Demonstrated excellent time management skills
Demonstrated ability to work within KPI's
Demonstrated high level of problem-solving skills and ability to work with minimal supervision
Demonstrated ability to understand and apply NDIS Practice Standards
Ability to manage risk, prioritise tasks, and meet deadlines under pressure
Excellent time management, discretion, and professionalism
Ability to develop and maintain effective working relationships with health and community service providers
A valid WA Driver's Licence
A current Police & WWC Checks and NDIS worker screening
