In this role, you will provide comprehensive medical care at one, and no more than two, high-quality long-term care communities in suburban Lancaster County, Pennsylvania .
You will work alongside a dedicated Penn State Health Advanced Practice Clinician at each site, delivering patient-centered care that improves outcomes and enhances quality of life for older adults and post-acute patients.
