Multiple permanent full time positions available
Task and Responsibilities
Provide personalised support and assistance to clients with disability in their homes and community settings, in accordance with their individual needs and support plans.
Assist clients with daily living activities, including personal care, meal preparation, household tasks, mobility, and medication support in accordance with relevant procedures and authorised care plans.
Support clients to maintain their independence, develop daily living skills, and achieve their individual goals.
Work collaboratively with clients, families, carers, and relevant health and support professionals to deliver appropriate and consistent care.
Encourage and support clients to participate in social, recreational, educational, and community activities to promote social inclusion and wellbeing.
Monitor clients' physical, emotional, and general wellbeing and promptly report any changes, concerns, incidents, or risks to the appropriate supervisor or relevant professional.
Maintain accurate and up-to-date client records, progress notes, incident reports, and other required documentation in accordance with organisational policies and confidentiality requirements.
Follow individual support plans, workplace procedures, health and safety requirements, infection control practices,
and relevant disability support standards.
Promote a safe, inclusive, respectful, and supportive environment that protects each client's dignity, privacy, choice, and independence.
Assist with maintaining a clean, safe, and suitable environment for clients and report any hazards or maintenance concerns promptly.
Qualification, experience and skills
1–2 years of previous experience working as a Disability Support Worker or in a similar care or support role.
Sound understanding of disability support, person-centred care (support tailored to the individual's needs), and the implementation of individual support plans.
Ability to work independently while also contributing effectively within a team environment.
Solid interpersonal and communication skills, with the ability to build positive and respectful relationships with clients, families, and colleagues.
Demonstrated empathy, patience, reliability, and a genuine commitment to supporting people with disability to achieve their individual goals and maintain their independence.
Relevant qualification such as Certificate III in Individual Support, Certificate III in Disability, or an equivalent recognised qualification.
Pay: $70,****** – $80,****** per year
