Swan Hill District Health is funded to provide CHSP service to support clients in their home. CHSP Personal Care and Relief Respite is facilitated and managed under District Nursing. We aim provide the best quality care to the clients and ensure that clients’ needs are met at all times.
CHSP client’s services are provided from Mon to Fri (0700hrs to 1530hrs) excluding public holiday.
We are currently looking for suitable candidate to join our dynamic team. The role is part –time 0.4 EFT fortnight.
