About the role

Facilitate rehabilitation, recovery, and transition to independent living. Work with the consumer, their families, carers and significant others with a focus on planning and provision of psychosocial care and therapeutic interventions within recovery and rehabilitation frameworks, and in alignment with the state-wide Community Care Unit (CCU) Model of Service.
