Established in 2002, Carbal Medical Services (Carbal) is a not-for-profit, charitable organisation dedicated to delivering comprehensive health services to the Aboriginal and Torres Strait Islander communities in the Toowoomba and Warwick regions. With a strong focus on culturally appropriate care, Carbal operates two medical practices and a wide range of outreach programs to support the health and wellbeing of First Nations people across the Darling Downs, Southern Downs, and Goondiwindi regions.

Position Overview

A key focus for Carbal is becoming a My Aged Care provider, delivering essential services to both Indigenous and non-Indigenous clients. We are seeking a passionate and experienced professional in the aged care sector to lead and provide exceptional care and support for some of our most vulnerable individuals, both at home and in the community.

The position in located in Toowoomba.
Main Duties and Key Responsibilities
- Lead, manage, and implement:
- Commonwealth Home Support Program
- Home Care Packages
- Short Term Restorative Care Program
- Elder Care Support
- Capable of recognising the differing care needs between Indigenous and non-Indigenous clients and understanding the complexities involved.
- Able to understand the differences in care needs for indigenous vs non-indigenous clients and the complexities involved.
- Ensure the delivery of culturally safe, client-centered aged care services of the highest quality.
- Oversee the daily operations of Carbal's aged care programs, ensuring seamless service delivery and compliance with regulations and standards.
- Oversee and manage rostering to ensure efficient service delivery and adequate coverage for all aged care programs.
- Supervise and support case managers, ensuring effective case management practices and regular reporting to maintain high standards of client care and program outcomes.
- Manage and monitor client finances and budgets to optimise individual health outcomes.
- Collaborate with the finance team to ensure a financially sustainable model, continuously adapting to changes for improved efficiency and effectiveness.
- Recruit and oversee subcontractor agreements, ensuring compliance with requirements and standards.
- Reduce employee and contractor turnover and enhance staff retention and engagement by managing workforce capacity and capability.
- Lead clinical governance and manage the quality management framework within Homecare services to ensure care standards are met.
- Foster strong collaboration with key stakeholders, including ACAT (Aged Care Assessment Teams) and RAS (Regional Assessment Service) teams, to ensure seamless service integration and coordinated care for clients.
- Conduct regular audits and reviews of aged care activities to ensure effective risk management and adherence to compliance requirements.
- Participate in Aged Care Quality and Safety Commission assessment and monitoring activities, and Program Assurance reviews.
- Manage complaints and incidents promptly, ensuring alignment with legislative obligations.
- Stay updated on industry standards, including the Aged Care Act and Aged Care Quality Standards, ensuring compliance with reforms.
- Ensure timely and accurate submission of reports and data through the DEX (Data Exchange) system to meet funding and compliance requirements.
Requirements:
Essential
- Proven leadership and operational capabilities within the Aged Care industry.
- Working knowledge of the principles of aged care funding principles including Home Care Package, Commonwealth Home Support Program and Short Term Restorative Care.
- Knowledge of the National Safety & Quality and Aged Care Quality Standards
- Knowledge of the Serious Incident Response Scheme
- Have knowledge and experience working with clients with various levels of aged care services access requirements.
- Excellent leadership and interpersonal skills
- AHPRA registration
- You must have an Open C Class driver license and possess a Blue Card and be willing to undergo a Police Check;
Desirable
- You will have a demonstrated understanding of the health and social issues found in our Indigenous Communities.
- Previous experience working in the Not-For-Profit sector.
- Experience in working effectively with Aboriginal and Torres Strait Islander people and a clear understanding of Aboriginal and Torres Strait Islander Culture.
