Each day, our team of dedicated Support Coordinators takes a proactive, responsive, and outcome-focused approach to meet the high standards we set for service delivery. The Role

As a CoAbility Support Coordinator, you will be pivotal in providing exceptional support services to Level 2 and 3 participants. Your work will directly impact and make a real difference in our participants' lives.

Your main responsibilities will include:

- Connecting participants with relevant services and supports
- Helping participants understand and make the most of their NDIS plan
- Helping participants navigate challenges in their support environment, such as access to education, health, justice services and similar
- Reducing barriers that prevent participants from implementing or using their NDIS plan as intended
- Helping participants understand and negotiate service agreements
- Helping build participant capacity and self-advocacy skills
- Developing and designing support solutions to meet participant outcomes




- Assisting in resolving points of crisis and developing capacity and resilience within the participant's network
- Coaching participants through challenges
- Collaborating with a variety of stakeholders
- Assisting participants in preparing for their plan review and reporting their achieved outcomes
- Modelling and reinforcing organisational values, culture and practice

The Opportunity

This role is for independent subcontractors, meaning you’ll be responsible for managing your own business operations, including insurance, tax, and other expenses.

- Referrals to build your caseload: Whilst you build your participant caseloads, our engagement team seek and receives referrals for you as well.

About You

To excel in this position, you will:

- Have a tertiary qualification in social work, human services, community services, or a related field.
- Bring experience in Support Coordination or Case Management.
- Be skilled at managing caseloads and meeting deadlines.
- Understand participant needs and help them achieve their goals.
- Thrive in a remote environment,



balancing multiple priorities effectively.
- Embrace cultural awareness and inclusivity in your practice.
- Communicate clearly and empathetically, both written and verbal.
- Have strong knowledge of the NDIS and relevant legislation.
- Be organised, process-oriented, and excellent at time management.
- Network effectively with local service providers.

You will need:

- 100 Points of identification.
- A valid Driver's license and comprehensively insured motor vehicle.
- Full Australian working rights.
- Australia Business Number (ABN).
- Valid NDIS Worker Screening Check.
- Relevant State’s Working with Children's Check.
- Public Liability and Professional Indemnity Insurance.
- Copies of relevant course certification.
- A can-do attitude.
