We are seeking a Support Worker to provide care and support our clients in the community. You will help clients achieve their goals and contribute to their learning and delivering of person-centred and strengths-based service.
THE ROLE
- Work with clients, family/carers and case managers to support the client’s individual goals.
- Support clients to participate in activities and tasks, in home support and community access that includes social activities, shopping/groceries, traveling to appointments, domestic assistance etc
- Ensure completion of appropriate documentation and all relevant information, on support provided, is to be recorded in client notes
- Provide administrative assistance and support to the team as required.
WHAT TO BRING
- Certificate or Diploma in Community Services/Mental Health or related discipline.
- Experience in community services and clients from cultural and linguistically diverse backgrounds.
- Proven highly developed organisational and time management skills.
- Highly developed communication and interpersonal skills.
- Basic proficiency in MS Office or similar software and experience using databases
- NDIS Worker Screening Check
- Current working with children check
- Relevant Vaccination Status
