About the role

AAP Community Services is seeking compassionate and dedicated Disability Support Workers to join our growing team. We are committed to delivering high-quality, person-centred support that empowers individuals living with disability to achieve their goals, maximise their independence, and actively participate in their communities. The successful candidate will have experience providing disability support services and a genuine passion for improving the quality of life of people living with disability. You will possess strong interpersonal and communication skills, demonstrate a person-centred approach to care, and have the ability to build positive and skilled relationships with participants, their families, and support networks. Experience supporting individuals with complex needs, personal care requirements, and community access activities will be highly regarded.

Key responsibilities

- Understand the individual needs, goals, preferences and lived experiences of participants and provide person-centred support

- Support participants in accordance with their functional abilities, support requirements and complex needs

- Empower participants to make informed decisions and exercise choice and control in all aspects of their daily lives

- Promote and uphold the dignity, privacy, confidentiality, independence and rights of all participants

- Develop and maintain safe, respectful and professional relationships with participants, their families and support networks through effective communication and active listening

- Assist participants with activities of daily living, including personal care, dressing, bathing, grooming, toileting, mobility assistance, meal preparation and eating as required

- Support participants to access community-based activities, social programs, education, training, employment opportunities and other disability support services to enhance social inclusion and independence

- Provide emotional support and assist participants to develop confidence,



life skills and greater independence

- Monitor participant wellbeing and promptly report any concerns, incidents or changes in support needs

- Maintain accurate records, progress notes and documentation in accordance with organisational and legislative requirements

About you

- Certificate III or IV in Disability Support, Individual Support or equivalent

- Current Working With Children Check (WWCC) and NDIS Worker Screening Clearance, or the ability to obtain and maintain these clearances

- Completion of the NDIS Worker Orientation Module

- Current First Aid and CPR Certification, or willingness to obtain prior to commencement

- Current unrestricted Australian Driver Licence

- Demonstrated flexibility to work across a 24/7 service environment, including weekends, public holidays, sleepover shifts, and active night shifts on a rotating roster

- Demonstrated experience providing support within the disability, aged care, or community services sector

- Sound understanding of the National Disability Insurance Scheme (NDIS), including participant-centred service delivery and relevant operational requirements

- Strong interpersonal, communication and relationship-building skills, with the ability to engage effectively with participants, families, carers and other stakeholders

- Demonstrated ability to work independently and collaboratively within a multidisciplinary team environment
