About the role
Newly created full time fixed term position until 30 September 2027 based in Glenside, SA. Reporting directly to the Team Leader Client Services, you will be responsible for:
- Facilitating individual and group education, information, and support to people living with dementia, their families, and carers. (this is the primary focus of the role)
- Supporting clients in navigating aged care, disability, and health services by providing referrals and guidance
- Facilitating connections between people living with dementia and their carers through group engagement activities
- Ensuring service delivery is person-centered, empowering clients to take an active role in their care
- Contributing to continuous improvement by identifying gaps in service delivery and sharing insights with leadership
- Building and maintaining partnerships with community organisations and service providers to enhance support options
- Maintaining accurate client records and data in compliance with professional and legal standards
- Regular intrastate travel (including to regional locations and the occasional overnight stay) is required as part of this role - fleet vehicle will be provided.

About you
To be successful in this role, we are looking for someone with the following:
1. Formal qualification in a related field of either Social Work, Allied Health, Nursing and/or significant experience working with clients in the delivery of a community-based service or within a health-related field
2. Demonstrated experience and understanding of the impacts of dementia for individuals, carers, and their support networks
3. Demonstrated experience and knowledge of group facilitation and group dynamics is essential as the primary focus of this role. Experience in facilitating individual and group work with clients in a community setting is desirable
4. Embraces new ideas and change initiatives
5. Achieves results through a commitment to quality, planning and efficient resourcing
6. Communicates clearly and openly with different audiences
7. Valid drivers licence required for intrastate travel.
