St Vincent's Health Australia is seeking a Social Worker to coordinate the Residential Aged Care Placement Service at St Vincent's Hospital Sydney. The role supports the multidisciplinary team to move patients to RACFs and improve outcomes.

The position is permanent part time (32 hours per week) with pro-rata salary, salary packaging, and NSW Health benefits. You will work with ward staff, participate in case conferences, and contribute to discharge planning and quality improvement.
