Across our renowned precinct, we feature cutting-edge programs and surgery infrastructure, fostering clinical excellence and professional growth.Located in the vibrant heart of Darlinghurst, we are positioned just minutes from Kings Cross station, with direct, high-frequency bus connections from the east, and seamless links from Gadigal and Museum stations right to our doorstep.Read on to explore this exciting role and the fantastic perks waiting for you here.About the roleCoordinate the Residential Aged Care Placement Service at St Vincent’s Hospital Sydney, supporting the multidisciplinary team to facilitate timely and efficient transfers of patients to Residential Aged Care Facilities (RACFs) whilst playing a pivotal role in improving patient outcomes, reducing hospital length of stay, and strengthening relationships with aged care providersMaintaining and updating the weekly RACF vacancy list locally and externally, and providing updates on placement progressConduct psychosocial assessments and coordinate discharges,



including providing support to patients and families/carers during the transition from hospital to RACFs and managing complex family dynamicsDevelop expert knowledge of Commonwealth-funded aged care services and build and maintain effective working relationships with Residential Aged Care Facilities (RACFs)Work collaboratively with ward social workers and multidisciplinary teams, including attending ward case conferences and family meetings as requiredMaintain documentation in accordance with professional standards, provide clinical handovers, maintain statistical records and generate monthly reports for presentation to senior managementAbout youQualifications in accordance with Schedule C of the NSW Health Service Health Professionals (State) Award, providing eligibility for membership of the Australian Association of Social Workers (AASW)Demonstrated skills and experience in psychosocial assessment, crisis intervention, counselling, advocacy and discharge planning, including the ability to participate in the After Hours Crisis On-Call Social Work ServiceDemonstrated experience and ability to work effectively as a member of multidisciplinary teams in a high-pressure workplace with competing demands, with strong negotiation and liaison skills, knowledge of aged care and community resources, and experience working with community organisationsWell-developed time management, prioritisation, computer, written and verbal communication skills, with the capacity to use health information systems effectivelyAbility to contribute to research projects, quality improvement and education activities within designated specialties and the Social Work ServiceWhat we can offer youExceptional Rewards: Competitive pay and not-for-profit salary packaging to maximise your incomeSeamless NSW Health Mobility: Smooth transfer of your qualifying service and leave entitlementsWorld-Class:
