About the opportunity:
Join a team dedicated to helping seniors live safely, confidentlyand independently in their own homes. As a Community CHSP CarePartner, you’ll contribute to creating and delivering personalisedsupport services aligned with Carinity’s Model of Care, upholdingdignity, wellbeing, and meaningful connection. You will beresponsible for supporting the operational set-up, day-to-daycoordination, documentation quality, and compliance readiness of acommunity business hub assisting elderly access entry levelCommonwealth Home Support Program services.

You will foster a safe,inclusive and respectful environment for clients, families, staffand visitors, while actively contributing to service improvementsand supporting the broader goals of the CHSP Community Care team.

This role requires availability Monday to Friday between 8:00 AMand 4:00PM.

Your day-to-day will involve:

- Coordinating each client’s journey with Carinity, frominitial meet‑and‑greet through admission, ongoing reviews,referrals. and discharge
- Supporting clients to develop, assess, and maintain budgetsand individualised care plans that meet each client’s approvedservices, unique needs, preferences and goals
- Ensure care services reflect a person-centred,strengths-based approach
- Monitor client wellbeing, processing referrals, assessmentsand care reviews.
- Provide guidance and liaise with Lifestyle Carers,contractors, and clients when required, contributing toscheduling, onboarding, and staff performance feedback




- Build strong relationships with clients, families,assessment teams, healthcare providers and internalcolleagues
- Advocate for clients and ensure any concerns or changes inconditions are followed up promptly and appropriately

About you:
You’re a warm, compassionateprofessional and community- minded individual who takes pride insupporting seniors to live well at home. You approach everyinteraction with confidence, warmth and professionalism whetherguiding staff, collaborating with families or coordinating withexternal providers. Highly organised and dependable, you seamlesslybalance administrative duties with hands-on support. Youcommunicate effectively with clients, families, staff and healthprofessionals, and you’re skilled at mentoring colleagues whilefostering a positive, collaborative team culture.

Additionally;

- Certificate III in Aged Care, Individual Support orCommunity Care
- Experience delivering or working within Consumer DirectedCare or Care management models
- Ability to monitor and support client budget processes
- Knowledge of Strengthened Aged Care Quality Standards,SIRS, Aged Care Act 2025
- Experience using Microsoft Office and care-relatedsoftware
- Current NDIS Worker Screening and a current driver’slicence
