Hunter Recent England Local Health District is seeking a Non-Clinical Assessor to perform home support assessments for older people under the Aged Care Act 2024. You will use the Integrated Assessment Tool (IAT) to identify suitable supports and develop individualised Support Plans focused on wellbeing and independence.

The role requires autonomous work in the community while collaborating with a multidisciplinary team to achieve positive outcomes.
