Western Health is seeking an experienced Clinical Nurse Consultant to join the Nursing at Home team in Melbourne. The role focuses on delivering high-quality, home-based care to support patients to stay healthy and protected at home.

You will perform advanced assessments, coordinate care and use telehealth and home visits to prevent hospital admissions. The position is part-time (40 hours per fortnight) with ongoing employment and opportunities for professional development.
