Community Carers NSW is growing, and we’re looking for an experienced Support Coordinator to join our friendly team in Caringbah.

The Role

Caringbah, NSW

Casual with an opportunity to transition to permanent employment for the right candidate

Hourly rate of $50–$70 (depending on experience)

What You’ll Do

- Coordinate NDIS supports and services
- Build strong relationships with participants and families
- Liaise with support workers and allied health professionals
- Monitor participant goals and service outcomes
- Maintain accurate records and case notes
- Support new participant onboarding




- Help participants access appropriate supports

About You

- 2+ years’ NDIS Support Coordination or Case Management experience
- Strong understanding of the NDIS
- Excellent communication and organisational skills
- Solid relationship-building and problem-solving skills
- Ability to work independently and as part of a team
- Level 1 or Level 2 Support Coordination experience is highly regarded

Essential

- NDIS Worker Screening Check
- Working With Children Check
- First Aid & CPR
- Driver Licence & reliable vehicle
- National Police Check or willingness to obtain
- Full Australian working rights
