Ozcare is seeking a compassionate Client Liaison Officer for its Hervey Bay Aged Care at Home team in a permanent part time capacity. You will coordinate client care, support intake, assessment and ongoing reviews, and act as the primary contact for clients and families.
You will build relationships, ensure service continuity, maintain records and support care planning while upholding aged care standards and organisational policies.
