Care Partner - Aged Care (Case Management)

About the role
You'll manage a portfolio of Support at Home and Claims Conference (Holocaust Government funding) participants, coordinating their care from planning through to review. Day to day, you'll be:
- Developing individualised care plans, built around each participant's goals, preferences, culture and risk factors.
- Managing client budgets — keeping spend on track and resources aligned to changing needs
- Coordinating across our Restorative Care, End-of-Life, and Assistive Technology & Home Modifications (AT-HM) pathways, working closely with clinically qualified staff when needs go beyond general care management
- Building genuine,
trusted relationships with participants and their supporters & educating & empowering them around aged care navigation & wellness.
- Liaising with GPs, allied health and other key providers and our aged care service coordinators & workforce planners.
- Keeping documentation sharp and compliant with the Strengthened Aged Care Quality Standards
About you
- Demonstrated experience in care coordination or case management in aged care or disability services
- A Diploma in Community Services, Aged Care, Case Management, or equivalent experience would be highly desirable.
- Solid understanding of the Support at Home program, Quality Standards and aged care rights
- Confidence interpreting clinical guidance and knowing when to escalate
- Comfortable working with client management systems and Microsoft Office
- A current NDIS Worker Screening Check and Working With Children Check (or willingness to obtain)
- Genuine empathy and cultural sensitivity — an understanding of and respect for the unique needs of Holocaust survivors and the Jewish community
Desirable: experience with the Claims Conference home care program, and knowledge of Jewish values, heritage and culture.
