As a Participant Program Coordinator, you’ll manage a caseload of TAC and NDIS participants, coordinating supports from onboarding to offboarding. You’ll build strong relationships, monitor wellbeing, ensure services stay within budget, and keep documentation accurate and up to date.
What You’ll Do
- Develop and review individualised support plans
- Liaise with families, guardians, clinicians and external providers
- Support and supervise Support Workers
- Manage incidents, risks and documentation
- Work closely with rostering and operational teams
What You’ll Bring
You are a naturally empathetic and organised individual, you can balance providing the best support for our participants and their families with partnering closely and getting to know our support workers.
Ensuring participants and their carers are expertly matched.
You are process driven and have a keen eye for detail, you care about the work you do and don’t lose sight of our patient centred values in times of pressure.
Experience in disability, aged care or community services
Robust communication and relationship‑building skills
Knowledge of NDIS/TAC frameworks and SCHADS Award
Ability to manage priorities in a fast‑paced environment
Tech confidence with care management systems
Requirements
- NDIS Worker Screening Check
- Working with Children Check
- Cert IV in Disability/Aged Care/Support Work (preferred)
- Driver licence and vehicle access
