In this role, you will:

- Build relationships with clients, understand their needs and goals, and act as their key contact.
- Coordinate and monitor services to ensure clients receive appropriate, person-centred support.
- Monitor changing needs, funding and safety, and arrange reviews or elevate concerns when required.
- Maintain accurate records and ensure services meet Support at Home and organisational requirements.
- Work with clients, families,



staff and providers to support positive outcomes.

Full- Time Monday - Friday orFlexible
