We’re looking for an experienced Registered Nurse to join our team as a Clinical Care Lead – a hands-on role for someone who loves being a nurse and wants to remain connected to clients, while also helping shape the quality and consistency of care around them.

This is a hybrid role blending both in person care and in office excellence. You’ll spend around two to three days each week out in the community, visiting clients in their homes and providing thoughtful, personalised clinical care and support in proactive clinical led care. The remainder of your time will focus on the clinical leadership activities that makes exceptional home care possible – supporting our team, overseeing care and helping ensure every client receives safe, considered and genuinely individual support.

About the role

As Clinical Lead, you’ll work closely with our Clinical Experience Manager and Care Coordinator to ensure our clients receive exceptional care at every stage of their journey with Sue’s Home Care.

Your responsibilities may include:

- Providing clinical care and assessments in clients’ homes.

- Developing, reviewing and overseeing individual care plans.

- Identifying changes in clients’ needs and ensuring care adapts appropriately.

- Providing clinical guidance, training and support to our Support Workers.

- Working closely with clients, families and the wider care team to ensure clear, consistent communication.

- Supporting the safe and effective delivery of complex care.

- Supporting of scheduling and care implementation activities.

- Supporting clinical governance, quality, compliance and audit readiness.

you’ll help make sure our clinical standards are not simply something we document – they’re something our clients experience.

About you

You’re an experienced Registered Nurse who combines strong clinical judgement with warmth, empathy and a genuine commitment to person-centred care.

You’re comfortable working autonomously in the community, but you also enjoy being part of a team-oriented team. You communicate clearly, notice the details and understand that excellent clinical care is as much about listening and building trust as it is about technical expertise.

You’ll bring:

- Current AHPRA registration as a Registered Nurse.

- Strong clinical assessment and decision-making skills.

- Confidence working independently in clients’ homes.

- Excellent communication and relationship-building skills.

- The ability to guide and support others with confidence and kindness.

- Strong organisation and clinical documentation skills.

- An unrestricted Australian driver’s licence and reliable vehicle.

- Experience with My Aged Care & NDIS desirable.

- A genuine commitment to helping people remain safe and independent at home.
