HPS is seeking a passionate and adaptable Clinical Pharmacist to support healthcare services across a correctional centre in Parkville, Melbourne working 9am to 2pm each Friday. This unique role offers the opportunity to work closely with multidisciplinary healthcare teams, providing medication management and clinical pharmacy services to a complex patient population.

What You'll Be Doing
- Conduct clinical medication reviews and medication reconciliation
- Provide expert medication advice to medical, nursing and healthcare teams
- Support safe and effective use of high-risk and controlled medicines
- Identify opportunities to improve medication safety and patient outcomes
- Participate in clinical governance and quality improvement activities
- Build strong partnerships with multidisciplinary healthcare stakeholders
About You
- Current AHPRA registration as a Pharmacist
- Robust clinical knowledge and patient-centred approach
- Excellent communication and stakeholder engagement skills
- Ability to work independently across multiple locations
- Strong organisational and problem-solving skills
