This is a rewarding opportunity for an experienced aged care or community services qualified who is passionate about supporting older people and their families through their journey into home care.

As the Home Care Client Onboarding Coordinator, you will be responsible for supporting potential clients and their families as they transition to becoming Goodwin clients. You will assess their needs, develop personalised care plans, coordinate services and help clients navigate the aged care system with confidence.

About the role

In this role, you will:

- Support potential clients and their families with information about Goodwin's Home Care services and available options.
- Guide clients through the onboarding process and support a smooth transition into Goodwin.
- Assess clients' needs, goals,



preferences and wellbeing in partnership with the client, their family and other relevant stakeholders.
- Develop, implement and review individualised care plans that promote client choice, independence, health and wellbeing.
- Support clients to establish realistic and achievable goals.
- Develop client budgets to maximise access to appropriate services and supports.
- Advocate for clients and liaise with families, guardians, medical professionals, allied health providers and other support services.
- Help clients navigate the aged care system and understand the services available to them.
- Identify when medical, emergency, allied health or specialist interventions may be required.
- Identify and coordinate appropriate equipment, aids and other supports.
- Maintain accurate and timely client records in accordance with organisational and legislative requirements.
- Monitor and evaluate services to ensure they are meeting clients' needs, goals and preferences.
- Work closely with the Home Care team to ensure high-quality services are delivered.
- Provide guidance, support and training to members of the service delivery team.
- Monitor Home Care Carers to support effective and high-quality service delivery.




- Contribute to continuous improvement and ongoing professional development.

About you

We are looking for someone who is compassionate, organised and confident in working with older people and their families.

To be successful in this role, you will have:

- Qualifications in Aged Care, Community Services or a similar field are highly desirable.
- Experience working with older people, preferably within aged care, home care or community services.
- Strong knowledge of ageing-related issues, including dementia and behaviour support.
- Experience in case management and assessing the care and support needs of older people living independently in their own homes.
- Excellent communication, negotiation and advocacy skills.
- Strong organisational, planning and problem-solving skills.
- The ability to work independently while contributing positively to a team setting.
- Strong computer skills, including proficiency in Microsoft Office.
- A high level of integrity and commitment to confidentiality.
- A genuine commitment to person-centred care and supporting client choice and independence.

Essential requirement

A current driver's licence and access to a reliable vehicle are essential for this role. The vehicle must have comprehensive insurance and be available for work purposes.
