We are seeking an experienced Home Care Client Onboarding Coordinator to support older people and their families as they transition into Goodwin Home Care.
In this role, you will assess client needs, develop personalised care plans, coordinate services and help clients navigate the aged care system, ensuring they receive the right support to maintain their independence, wellbeing and quality of life.
About the role
You will:
- Support potential clients and families through the Goodwin onboarding process.
- Assess client needs, goals and preferences and develop individualised care plans.
- Advocate for clients and liaise with families, guardians,
health professionals and support services.
- Develop client budgets and coordinate appropriate services, equipment and supports.
- Help clients navigate the aged care system and access the right care.
- Monitor and review services to ensure they continue to meet client needs.
- Maintain accurate client records and support quality and compliance requirements.
- Work closely with the Home Care team and provide guidance to service delivery staff.
- Contribute to professional development and continuous improvement.
About you
You will be a compassionate, organised and confident professional with:
- Qualifications in Aged Care, Community Services or a related field highly desirable.
- Experience in aged care, home care, community services or case management.
- Robust understanding of ageing, including dementia and behaviour support.
- Excellent communication, advocacy, negotiation and interpersonal skills.
- Strong organisational and problem-solving skills.
- Competent Microsoft Office skills.
- A person-centred approach and commitment to supporting independence and choice.
Essential
- Current driver's licence and access to a reliable vehicle with comprehensive insurance.
