Pearl Home Care Newcastle and Hunter is seeking an experienced and compassionate Clinical Care Partner to join the local team and help older Australians live safely, independently and confidently at home under the Support at Home program. The role involves managing Support at Home consumers across the Newcastle and Hunter region, building trusted relationships with consumers and their families, and providing clinical oversight for those with complex care needs.
Key responsibilities
Managing Support at Home consumers and developing robust, trusting relationships with them and their families
Developing, implementing and regularly reviewing individualised care plans
Providing clinical oversight for consumers with complex care needs
Maintaining comprehensive clinical and care-management documentation
Liaising and coordinating services with support workers, nurses, allied health professionals, GPs and other providers
Supporting the delivery of safe, high-quality support at home services in accordance with the Aged Care Act, Aged Care Quality Standards, Code of Conduct for Aged Care and Support at Home requirements
Supporting consumers to navigate the aged care system and access appropriate services and supports
About you
Current support worker/ home care coordination / nursing qualification (Degree/ Diploma/ Cert III–IV or equivalent) and relevant experience.
National Police Check completed within 12 months
Current First Aid and CPR certification
Current Australian Driver Licence and reliable, comprehensively insured vehicle
Strong computer literacy and confidence using care-management and clinical documentation systems
Australian working rights
Experience in aged care, community nursing, home care, care coordination or a similar community-based clinical role
Ability to work autonomously in consumers' homes and across the community
