Located across the Eastern, Outer Eastern and Yarra Valley regions. Office based in Ferntree Gully. Travel required.

As a Support Worker, you’ll play a vital role in enhancing the independence and well-being of our clients. Your responsibilities will include:
Providing Personalised Support: Deliver high-quality, individualized assistance that empowers older adults to live fulfilling lives.
Creating Connections: Offer companionship and emotional support through engaging conversations and meaningful interactions.
Facilitating Independence: Help clients with personal care and transportation to appointments,



ensuring they stay connected and active in their communities.
Implementing Care Plans: Follow and execute individual service, healthcare, and positive behavior support plans, tailoring your approach to each client's unique needs.

Who we are looking for
If you have experience working as a Support Worker, are qualified with a Minimum Cert III in Aged Care, Community Support or related field, and enjoy variety in your day, you might be exactly the person we're looking for!
Minimum Cert III Qualification in Aged Care, or Individual Support (must have completed aged care components).
Experience working with older adults.
The ability to follow and implement individual service plans, healthcare, and positive behaviour support plans.
A desire to use your skills and experience creating fun and interesting activities that support older adults to engage more consistently with their community and peers.
A valid driver’s licence.
