Cafs in Ballarat is seeking a Leaving Care Housing Practitioner for a full time role focused on securing safe, secure housing for young people transitioning from care. You will work with care teams, housing providers and community services to develop housing pathways aligned with wellbeing and transition goals.
The role requires robust advocacy, system navigation and engagement skills to support young people with complex needs, including trauma, mental health challenges and AOD concerns.
