The Clinical Care Case Manager is responsible forthe management and delivery of clinical support to teams across CHSP, HACC and HCP within a Consumer Directed Care framework.The Clinical Case Manager will focus on delivering support and leadership to the Home Care team across Cairns regionThe organisation will contribute to developing you into a Service Management position$140,000 + Super + Salary PackagingAbout the roleThe Community Home Care Clinical Case Manager will help to ensure high standards of the services are person centered, responsive and support clients to live as well and independently as is possible in their own home and community based across the Cairns region.To help achieve this purpose, the Clinical Case Manager will provide effective support and leadership to the team and focused on delivering high-quality, person-centred care and solid clinical governance across the region.DutiesLead the Clinical team to manage CHSP, HACCandHCP programs to ensure appropriate services are delivered that meet clientneeds.Ensure services meet Aged Care Quality Standards, legislative requirements and organisational policiesLead clinical risk management, including incident oversight,
