Our values of respect, compassion and excellence guide everything we do, and we pride ourselves on delivering person-centered care that honours the unique needs of each individual in our community.About the roleWe are seeking a NURSE Care Coordinator, to join our team at the Sacred Heart Monastery Assisted Living facility on a part-time basis.
This is a vital position within our senior care operations that sits at the heart of the care delivered.
As the Care Coordinator, with the other nursing staff and carer, you will play a pivotal role in ensuring seamless coordination of care services, supporting both the Priests and Brothers.
You will enjoy the role caring for well seniors, making this a highly rewarding and meaningful role.The Missionaries of the Sacred Heart is committed to safeguarding all people, particularly children and adults at risk and has a zero tolerance to abuse.Key responsibilitiesCoordinate and schedule care appointments, ensuring best care is arranged, medications delivered as ordered and documented.Communicate effectively with care staff, nurses, allied health professionals and external service providers to ensure quality care delivery.Assess residents' changing needs and coordinate appropriate responses, liaising with the Director of Care and RN, to adjust care provisions as requiredParticipate in handover meetings and team briefings to ensure continuity of careIdentify and escalate any issues or safeguarding concerns to appropriate managementContribute to a positive,



enjoyable and inclusive workplace culture that reflects our values of compassion and respectWhat we're looking forA recognised NURSE/EEN/AIN qualification/sDemonstrable experience working in an aged care or healthcare setting, has a drivers license (ESSENTIAL) and willingness to drive and support seniors in an appointment and follow-up on any changes made.A caring person who enjoys providing best care in a faith based organisation.Excellent organisational and time management skills with the ability to manage multiple tasks and prioritiesOutstanding communication skills, both written and verbal, with the ability to liaise effectively with diverse stakeholdersProven ability to work collaboratively within a multidisciplinary team environmentCompassion, empathy and a genuine commitment to person-centred care and resident wellbeingComputer literacy and proficiency with electronic care management systems (desirable)Current First Aid and CPR certification (or willingness to obtain)A commitment to our faith-based values and inclusive, respectful workplace cultureWhat we offerWe are committed to supporting the wellbeing and professional development of our staff.
