We're building the foundations, systems, partnerships and people to grow into a provider that supports communities and people right across Australia, and we're looking for a Care Manager who wants to help build that, not just join it.About the RoleReporting to the Operations Manager, you'll own the day-to-day of how care actually gets delivered.
The role that keeps clients, support workers and our growing Delivery Partner network all moving in the same direction.You'll be responsible for:Overseeing day-to-day service delivery across our NDIS, Aged Care and Veteran (DVA/VHC) programsManaging rostering, performance and supporting our support workers and Delivery Partner networkLeading care and service plan coordination, and acting as the first point of contact for clients and families on service delivery mattersManaging NDIS Plan Management - processing and monitoring participant plan budgets, invoicing accuracy and reportingOwning compliance and quality at the coordination level - NDIS Practice Standards, Aged Care Quality Standards, incident management and documentationPartnering with the Operations Manager on service improvements,



systems and the initiatives that support our growth - including expanding our Delivery Partner network as we take on new regionsHelping shape how a small, values-led provider scales into a much bigger one, without losing what makes us good at what we do.
A personable experience.About YouExperience in care coordination, case management or client/service management within NDIS, Aged Care or veteran/DVA-funded programsWorking knowledge of NDIS Practice Standards and/or Aged Care Quality StandardsExperience with NDIS Plan Management or financial administration of client budgets is highly regardedProven ability to lead, roster and support a team of support workers or care staffStrong relationship-building skills with clients, families, Delivery Partners and referrersA relevant qualification in community services, health, social work, business or a related field (or equivalent experience)Current driver's licence, NDIS Worker Screening Check / Working with Vulnerable People Check and National Police Check (or willingness to obtain)Genuine ambition to grow with Peppermint CareWhat's on Offer$85,000 - $95,000 + Superannuation, dependent on experienceA real growth path: as Peppermint Care expands nationally, this role grows with itDirect support from an Operations Manager who's focused on supporting you and driving growthThe chance to help build the operating model for a multi-state care provider from the ground upA small-business culture with big ambitions - versatile, values-driven, and genuinely invested in the people who work here as much as the people we support
