This excellent opportunity will be offered on a Permanent Full-Time basis at 38 hours a week.BenefitsProfessional growthA friendly team workplace with a great community care spiritAbility to visibly see your impact on the business and its serviceCaura Pty LtdThis excellent opportunity will be offered on a Permanent Full-Time basis at 38 hours a week.BenefitsProfessional growthA friendly team environment with a great community care spiritAbility to visibly see your impact on the business and its serviceTask & responsibilitiesAssess participants' needs and develop, implement, and review individual support plans aligned with their NDIS goals.Meet with participants and their families to identify support requirements, strengths, barriers, and personal goals.Monitor participants' progress, maintain accurate case notes, and prepare reports to ensure supports remain effective and outcome-focused.Coordinate referrals and connect participants with appropriate health professionals, allied health providers, community services, housing, employment, and other support agencies.Assist participants to access NDIS-funded supports, community programs, education, employment, and independent living opportunities.Liaise with participants, families, carers, service providers, government agencies,
and community organisations to ensure coordinated and person-centred service delivery.Support participants with community participation, capacity building, life skills development, and social inclusion to promote independence and wellbeing.Provide guidance and advocacy to participants in navigating the NDIS, implementing support plans, and achieving their individual goals.Assist participants to overcome social, emotional, behavioural, and practical challenges through tailored support and mentoring.Prepare participant progress reports, service documentation, funding reviews, and other records required by the NDIS and relevant stakeholders.Qualification & experienceExcellent English skillsThe right to work in AustraliaAQF Associate Degree, Advanced Diploma or Diploma (ANZSCO Skill Level 2)in relevant fieldMinimum of 1 years of work experienceTeam management skillsAbility to work in a fast paced environmentStrong time management skillsAbout CompanyCaura Pty Ltd is a registered NDIS provider delivering person-centred disability support services across New South Wales.
