At Sundale, we live by our values:Care: Creating quality and meaningful experiences for everyone in the Sundale communityConnection: Listening and responding to the needs of othersCreativity: Finding better and more creative ways to do thingsWe offer a supportive, people-first workplace where your contribution truly matters.About the RoleBased at Sundale's Support Office in Nambour, the AN-ACC Coordinator RN works closely with the AN-ACC Manager and site-based teams to support accurate assessments, clinical documentation, compliance and funding outcomes.The position includes travel to Sundale sites to conduct clinical reviews, support AN-ACC Assessor visits and provide education and guidance to clinical and care teams.What You'll DoReview residents to identify opportunities for AN-ACC reassessmentSupport accurate assessments, care plans, progress notes and clinical documentationAttend and support scheduled AN-ACC Assessor visitsProvide AN-ACC education, coaching and guidance to clinical and care teamsMonitor clinical review data, funding projections and assessment outcomesPrepare reports and provide insights to support decision-makingConduct moving on audits (MOA) across residential aged care and in-home care servicesIdentify documentation gaps,
