Gladstone Region Aboriginal and Islander Community Controlled Health Service Limited (GRAICCHS), trading as Nhulundu Health Service, provides a comprehensive range of primary health care, aged care and community support services across the Gladstone and Central Queensland region.We are seeking a motivated experienced Registered Nurse who is passionate about making a difference in health outcomes for the Aboriginal and Torres Strait Islander Community, to join our multidisciplinary team and support the delivery of high-quality, culturally safe primary health care services to Aboriginal and Torres Strait Islander peoples and the wider community.Why Join Us?

you will provide comprehensive primary health care services while working closely with GPs and a multidisciplinary team to improve the health and wellbeing of our community.More specifically, this role will focus on key activities, including but not limited to:Deliver comprehensive primary health care services including triage, health assessments, wound care, immunisations and routine clinical procedures.Support chronic disease management through client assessments, care planning, monitoring and follow-up.Provide health promotion, education and preventative health initiatives to individuals, families and the broader community.Coordinate referrals and collaborate with GPs, specialists, hospitals and other health providers to ensure continuity of care.Maintain safe clinical practices, including infection control, medication management and vaccine cold chain management.Ensure accurate clinical documentation and support Medicare billing requirements.Participate in quality improvement initiatives and contribute to the ongoing enhancement of service delivery.Work collaboratively within a multidisciplinary team to achieve positive health outcomes for clients.Contributing to a positive team environment by mentoring students and colleagues,



participating in quality improvement activities and maintaining professional standards.Desired Skills and ExperienceWhat we are looking for:You are a compassionate and motivated Registered Nurse who enjoys working in a community-focused environment and values culturally safe care, who meet the following criteria:Current AHPRA registration as a Registered Nursewith no conditions or undertakingsA bachelor's degree in Science - Nursing (or higher)Demonstrated clinical experience in Primary Health Care with strong assessment and nursing skillsExperience in clinical or primary health care nursingStrong clinical assessment and communication skillsA commitment to culturally respectful practiceCurrent CPR and First Aid certificationA current Queensland C Class Driver LicenceA satisfactory Criminal History CheckA current Blue Card (or willingness to obtain)Experience working within a primary health care or Aboriginal Community Controlled Health Service environment.Immunisation endorsement and experience.Experience in chronic disease management and health promotion programs.Experience using electronic patient record systems and other clinical software.Pre-employment checks, including a National Criminal History Check, LCS2 and reference checks, will be required.Ready to join a team committed to improving access to health services for Aboriginal and Torres Strait Islander communities?
