ABOUT THE ROLEIf you're looking for a meaningful role where you can see the impact you make every day join our skilled, supportive, values driven multidisciplinary team based in Warabrook.As an Aboriginal Peer Navigator you will connect with Aboriginal and Torres Strait Islander people living with complex mental health and psychosocial challenges in the Newcastle and Hunter region.
Make a meaningful difference by walking alongside community to navigate health and community systems, build on their strengths, connect with cultural supports and work towards social and emotional wellbeing goals.DUTIESProvide culturally secure peer navigation and psychosocial support to Aboriginal and Torres Strait Islander participants.Connect participants and families with GPs, clinical, cultural community-based supportsBuild trusted relationships with participants, families and carers to support informed choices and wellbeing planningProvide practical support,
outreach and support coordination for participants and their support networks with the Yudhilidin teamMaintain accurate client records and contribute to continuous improvement of the programSKILLS & EXPERIENCEDemonstrated experience providing culturally safe support to Aboriginal and Torres Strait Islander peopleRelevant mental health experience, lived experience and/or qualifications (min. Certificate IV or working towards)Ability to connect, engage and support people within a social and emotional wellbeing frameworkExperience collaborating with external service providers, particularly primary care and other community-based health servicesStrong task management skills; a positive, proactive and solutions-focused attitudeHunter Primary Care offers you:Generous salary packaging up to $15,899 per annumPaid parental leaveProfessional development leave, study leave and clinical supervisionPaid mental health leave and cultural leaveEmployee Assistance ProgramThe successful applicant for this position will be asked to consent to a police check and will be required to hold a current NSW Working with Children check.
