goals and budgetsMonitoring client wellbeing and identifying emerging risks or support needsWorking closely with health professionals and community services to achieve positive outcomesManaging a diverse caseload while maintaining accurate records and documentationResponding to client feedback, incidents and concerns with professionalism and empathyDelivering exceptional customer service while supporting older people to live independently at homeAbout youYou are passionate about supporting older people to live independently and achieve their goals.
You enjoy building meaningful relationships, navigating complex situations and delivering high-quality, person-centred support.We're looking for someone with:Experience managing client caseloads and delivering person-centred, goal-focused support within the aged care, health, community services, allied health or disability sectorsExperience conducting assessments, developing care plans, coordinating services and reviewing client outcomesA strong understanding of Consumer Directed Care principles and person-centred practiceExcellent communication and relationship-building skills, with the ability to work collaboratively with clients, families, health professionals and service providersStrong organisational skills and the ability to manage competing priorities while maintaining accurate records and documentationA sound understanding of compliance, privacy, safety and skilled responsibilities within a care environmentThe ability to work independently while contributing positively to a collaborative team environmentA commitment to our BEST philosophy of Believe, Excel, Support and TrustYou must have, or be willing to obtain:Current First Aid and CPR certificatesA Victorian Driver LicenceAccess to a reliable, registered and insured vehicle for work-related travelA Victorian Working With Children Check (Employee)Current and ongoing flu vaccinationA National Police CheckWe warmly welcome applications from Aboriginal and Torres Strait Islander peoples.Help older people live well with choice and dignity - choose a career with heart.
