In this rewarding full time role, with an annual salary ranging from A$79,000 to A$82,000, you will play a vital role in supporting our clients to achieve their personal goals and enhance their daily living skills.
Your day-to-day responsibilities will naturally align with standard community support practices, ensuring person-centred care and building meaningful connections within the Darwin community.
Key responsibilities in this position generally include:
Providing direct,



person-centred support to individuals with disabilities in alignment with their individual support plans.
Assisting clients with daily living activities, community access, and capacity-building initiatives that promote independence.
Collaborating closely with clients, families, and multidisciplinary team members to ensure holistic and effective care.
Maintaining accurate case notes, documentation, and reporting in accordance with organizational policies and standards.
Fostering a safe, positive, inclusive, and empowering environment for all participants.
