Chosen Family in Recent South Wales is seeking a compassionate Community Support Worker to assist a 30-year-old gender-diverse participant with autism and related conditions to live independently and participate in the community on their terms.
You will provide trauma-informed, person-centred support, build genuine connections, and respect the participant's autonomy, values, beliefs and relationships while supporting transportation and social activities across Cambridge Park and surrounding areas.
