Open Minds in Penrith is seeking a Change Facilitator to support individuals in building independence and confidence. You will map out what matters to clients and co-create change plans with their goals, guiding progress and reporting outcomes.
You will liaise with mental health services and stakeholders to strengthen wellbeing, coordinate plans, and ensure compliant service delivery while maintaining confidentiality and cultural sensitivity.
