Youth Caseworker (SHS, Education & Employment)
Path 2 Change (P2C)
is seeking an experienced, passionate and youth-focused
Youth Education & Employment Caseworker
to join our team at Path 2 Change under the
Youth Initiative Program (YI)
.
This is a unique opportunity to work alongside young people who have experienced
Out of Home Care (OOHC), homelessness, housing instability and significant barriers to education and employment
, supporting them to build the skills, confidence and connections they need to create a more positive future. The
Youth Initiative's distinctive hybrid model
brings together three complementary roles – the
Education & Employment Mentor
,
Personal Advisor
and
Transitional Support Worker
– to provide young people with truly integrated support as they move towards independence. The
Education & Employment Mentor
focuses on helping young people identify and achieve their education, training and employment goals,
At P2C, we take a
trauma-informed, strengths-based and genuinely youth-centred approach
. We work with young people where they are, develop individual pathways around their goals and celebrate progress – whether that is enrolling in TAFE, obtaining a licence, completing a course, attending an interview, securing their first job or taking the next step toward independence.
As a Youth Education & Employment Caseworker, you will work directly with young people aged approximately *****, including young people:
Transitioning from Out of Home Care
Experiencing homelessness or housing vulnerability
Living in temporary, transitional or crisis accommodation
At risk of disengaging from education
Experiencing barriers to employment
With complex or competing support needs
Who may have had difficult or negative experiences with services in the past
You will build strong, trusting relationships and work alongside each young person to develop a practical
education, training and employment pathway
that reflects their strengths, aspirations and circumstances.
What you will do:
You will:
Provide individual case management and education/employment support.
Develop and regularly review individualised Education and Employment Plans.
Support young people to identify their goals, strengths, interests and career aspirations.
Connect young people with TAFE, education providers, training organisations and employment opportunities.
Support young people with resumes, applications, interviews and workplace preparation.
Assist young people to obtain qualifications, tickets, licences and other employment-readiness requirements.
Advocate for young people when navigating education,
employment, housing and other services.
Build strong relationships with employers, training providers, community organisations and other services.
Support young people to overcome practical barriers such as transport, identification, digital access and confidence.
Work collaboratively with OOHC providers, Specialist Homelessness Services and other key stakeholders.
Maintain accurate case notes, outcomes and reporting in accordance with program and funding requirements.
Participate in case conferences, team meetings and professional development.
Most importantly,
see the young person beyond their circumstances and support them to see possibilities for their future.

The Youth Initiative also works closely with P2C's broader youth programs, creating opportunities to connect young people with practical supports and pathways that extend beyond traditional case management.
About you:
Passionate about working with young people and achieving meaningful outcomes.
Experienced in youth work, case management, employment services, education support, homelessness or a related field.
Confident working with young people experiencing complex and multiple barriers.
Able to build genuine relationships based on trust, respect and consistency.
Trauma-informed and strengths-based in their approach.
Creative and solution-focused.
Confident advocating for young people and navigating complex service systems.
Organised and able to manage casework, documentation and competing priorities.
Comfortable working independently while being an active member of a small, supportive team.
Able to build strong relationships with employers, education providers and community services.
Committed to cultural safety,
inclusion and self-determination.
Experience working with
young people leaving OOHC and/or young people experiencing homelessness
will be highly regarded.
Essential Criteria:
Relevant qualification in Youth Work, Community Services, Social Work, Education, Employment Services or a related discipline, or equivalent experience.

Demonstrated experience working with young people experiencing disadvantage, homelessness, OOHC or complex circumstances.
Demonstated case management experience.
Demonstrated experience & knowledge of local Specialist Youth Services including
Specialist Homelessness Services (SHS)
, OOHC, Disability, Counselling, Drug & Alcohol, Mental Health, Health, and Legal services and thier operations/services.
High level of professionalism with excellent team, organisational, interpersonal and communication skills.
Interviewing, accidental counselling, advocacy, negotiation, networking, and problem-solving skills.
Writing skills- computer data entry onGovernment reporting portal, including Case plans, Case notes and Report writing skills.
Highly developed teamwork skills and a demonstrated ability to work independently.
Demonstrated case management experience.
Current Working With Children Check.
Current National Police Check or ability to obtain one.
Current Australian driver's licence.
Ability to travel across the Hunter/Newcastle region as required.
