We are seeking a passionate and experienced Registered Nurse to join our growing team and deliver high-quality nursing services to NDIS participants and other community clients in their homes and community settings.
About the Role
As a Community Registered Nurse, you will provide clinical assessment, planning, education, and nursing care to NDIS participants living in the community. Working closely with participants, families, support workers, allied health professionals, GPs, and other stakeholders, you will help ensure participants receive safe, coordinated, and responsive care that supports their health, wellbeing, and individual goals.
This role offers flexibility, autonomy, and the opportunity to build meaningful relationships while making a positive impact in the lives of people living with disability and complex health needs.
Key Responsibilities
Conduct intake nursing assessments.
Develop and review care plans.
Complete risk assessments and documentation.
Deliver clinical nursing care.
Monitor health and escalates concerns.
Provide health education and support.
Coordinate care with multidisciplinary teams.
Support independence and community participation.
Apply a person-centred approach.
Maintain compliance with NDIS and nursing standards.
About You
Essential Requirements
Current registration as a Registered Nurse (RN) with AHPRA
Demonstrated experience in community nursing, disability services, primary health care, aged care, rehabilitation, or a similar setting
Strong clinical assessment, care planning, and clinical decision-making skills
Experience delivering nursing services in home or community environments
Excellent communication, interpersonal, and documentation skills
Ability to work independently while collaborating effectively within a multidisciplinary team
Commitment to person-centred, strengths-based, and culturally responsive practice
Current First Aid and CPR certification
Valid Australian Driver's Licence and access to a reliable insured vehicle
Current NDIS Worker Screening Check (or willingness to obtain)
Full Australian working rights
Highly Regarded
Experience supporting NDIS participants with complex health needs, disability, or psychosocial supports
Knowledge of the NDIS Quality and Safeguards Framework and NDIS Practice Standards
Experience developing health support plans and clinical documentation for NDIS participants
Experience training and guiding disability support workers in health-related supports
Knowledge of positive behaviour support, restrictive practices, and participant safeguarding principles
Experience using electronic clinical records and care management systems
