Care ManagerNorth West Sydney | Residential Aged CareA well-established not-for-profit residential aged care provider in North West Sydney is seeking an experienced, people-focused Care Manager to join their team.This is an excellent opportunity for a clinically capable and supportive leader who enjoys working closely with Facility Managers, RNs and care teams to deliver high-quality outcomes for residents.You will join a medium-sized nursing home with an established leadership structure and further support from dedicated Quality and Education teams.If you are an experienced Care Manager who leads through communication, consistency and clinical excellence, this role offers the opportunity to make a genuine impact within a respected not-for-profit provider.About the RoleAs the clinical leader supporting the home, you will provide direction to the RN and care teams, oversee clinical standards and work closely with the Facility Manager to ensure residents receive high-quality, person-centred care.The role combines clinical leadership, team development, quality and compliance, and day-to-day operational oversight within a supportive and established environment.Key ResponsibilitiesLead and support the RN and care team to deliver high-quality, person-centred careOversee clinical care planning, documentation and medication managementMonitor clinical indicators, incidents and risks,
