Working closely with children, families, carers, Child Protection and community services, TCPs help create stable, nurturing and sustainable care arrangements that improve long-term outcomes.About the RoleAs a Targeted Care Packages Key Worker, you will:Provide case management and care coordination for children and young people with complex needs.Build strong relationships with children, families, carers and key stakeholders.Develop and implement support plans aligned to each young person's goals and best interests.Promote placement stability, connection to family, education and community.Deliver culturally responsive, trauma-informed and therapeutic practice informed by TBRI.Work collaboratively with Child Protection, carers,
schools and service providers to achieve positive outcomes.SW4 OpportunitiesThe SW4 role is ideal for practitioners who enjoy direct case management and working collaboratively within a multidisciplinary team to support children and young people to thrive.SW5 OpportunitiesThe SW5 role is suited to experienced practitioners who can manage more complex cases, provide guidance to colleagues and contribute to high-quality practice across the program.Why Join Cafs?
